  • We are looking for a skilled and passionate Band 6 Occupational Therapist to join one of the Trust in South West England. Trust provides a wide range of health services, including acute care, community services, mental health, and learning disability support. As an integrated trust, it is committed to providing high-quality, patient-centred care across hospital and community settings.

    The Trust only works on Direct Engagement Mode of payment.

    Description:

    This role involves working with musculoskeletal (MSK) patients and inpatients, focusing on rehabilitation, discharge planning, and admission prevention.

    Key Responsibilities:

    • Assess and evaluate patients in medical, surgical, and MSK settings.
    • Play a critical role in discharge planning to ensure safe and effective transitions.
    • Provide rehabilitation services aimed at promoting recovery and preventing readmission.
    • Manage a diverse and complex caseload independently, tailoring treatment plans to patient needs.
    • Collaborate with the multidisciplinary team to deliver high-quality, patient-centred care.
    • Conduct regular assessments, track progress, and adjust rehabilitation plans as necessary.

    Requirements:

    • Proven experience in medical, surgical, and MSK settings.
    • Strong skills in patient assessment, discharge planning, and rehabilitation.
    • Ability to manage complex cases independently.
    • Experience in an NHS or equivalent healthcare setting.
    • Excellent communication and collaboration skills for multidisciplinary teamwork.
