    We are looking for a dedicated and skilled Band 6 Occupational Therapist in South London for one of the Trust dedicated in providing high quality care. The Trust is one of the UK's most prestigious and well-established NHS Foundation Trusts, renowned for its excellent patient care, innovative services, and commitment to advancing healthcare.

    Job Description:

    As a Band 6 Occupational Therapist, you will be responsible for delivering comprehensive therapy assessments, interventions, and discharge planning for patients in various community settings. You will be working closely with patients and other healthcare professionals to ensure high standards of care and positive patient outcomes. Your responsibilities will include:

    • Conducting patient-facing therapy assessments, risk assessments, and clinical reasoning to develop tailored intervention plans.
    • Setting therapy goals and planning interventions to support patient recovery and independence.
    • Planning and coordinating safe and effective discharges.
    • Prescribing, reviewing, and monitoring equipment to meet patients' needs.
    • Working autonomously and effectively in a community setting, including urgent and rapid response environments.
    • Utilizing EPIC and other electronic health record systems for documentation and care planning.
    • Complying with the standards set by the Health & Care Professions Council (HCPC) and other relevant professional bodies.

    Requirements:

    • Professional registration with the Health & Care Professions Council (HCPC).
    • Experience in urgent/rapid response and community-based settings.
    • Competence in risk assessments, clinical reasoning, and lone working.
    • Proficiency in using EPIC or similar electronic health record systems.
    • Excellent communication, time management, and teamwork skills.
