  • We are looking for an Occupational Therapist to provide occupational therapy assessment and treatment interventions for a specified service user group, manage a caseload as a member of the multidisciplinary team (MDT), and develop the occupational therapy service provision within the work setting. Hertfordshire Partnership NHS Foundation Trust (HPFT) is a leading provider of mental health and learning disability services in East Anglia.

    Description:

    • Plan and implement individual and/or group interventions in collaboration with service users, using graded activity to achieve therapeutic goals.
    • Conduct occupational therapy assessments for a designated caseload, addressing occupational performance and skill deficits to enable service users in areas of self-maintenance, productivity, and leisure.
    • Build on existing knowledge of the Model of Human Occupation (MOHO) through the use of standardized assessment and intervention tools.
    • Following assessment, undertake case formulation to highlight occupational strengths and limitations and inform the intervention plan.
    • Work collaboratively with service users to identify OT goals as part of the overall recovery plan.
    • Provide interventions in a manner consistent with Trust standards and values.
    • Routinely provide feedback to the MDT regarding observed occupational performance throughout contact, informing ongoing assessment and therapeutic intervention.
    • Provide support and education to service users and carers regarding aspects of occupational therapy, such as equipment fitting and rehabilitation programs.

    Skills:

    • Strong clinical skills in assessing and treating a range of conditions.
    • Ability to develop and implement effective rehabilitation and discharge plans.
    • Excellent communication and teamwork abilities.
    • Experience with community-based pathways is advantageous.
