Key Responsibilities:

• Conduct assessments for highly complex patients within the Continuing Healthcare Team across the CTM Health board.
• Perform evaluations in patients' homes to determine the need for specialist manual handling equipment or techniques.
• Submit equipment requests to the CHC Panel for review and approval.
• Develop and implement tailored intervention plans for patients based on assessment findings.
• Collaborate with multidisciplinary teams to ensure integrated and effective patient care.

Qualifications:

• Registered Occupational Therapist with a valid HCPC registration.
• Proven experience in managing complex cases and providing high-quality occupational therapy services.
• Strong knowledge of manual handling techniques and equipment.
• Excellent communication skills and the ability to work effectively with patients, families, and healthcare professionals.

Desirable Skills:

• Experience working within a Continuing Healthcare Team or similar setting.
• Familiarity with the CTM Health board's policies and procedures.
