Direct Pay Mechanism

£23 - £25 per hour

Role Overview: We are seeking a dedicated and experienced Band 6 Occupational Therapist to join our community and rapid response teams. This role involves providing high-quality assessment and rehabilitation to patients in their homes across all 6 PCNs.

Key Responsibilities:

• Conduct comprehensive assessments of patients requiring rehabilitation in their homes.
• Provide rapid response for patient admission and urgent needs.
• Perform moving and handling risk assessments and prescribe necessary equipment.
• Liaise effectively with social services, continuing health care teams, equipment providers, and the multidisciplinary team (MDT).
• Adapt flexibly to rapid response requests and varying patient needs.
• Deliver care and support across three main locations.

Requirements:

• Must be willing and able to cover all sites in (Epsom, ICP Bourne Hall Health Centre, Molesy, Banstead, Leatherhead, East Elmbridge, Dorking).
• Car driver with access to a vehicle is essential.
• Strong competency in moving and handling practices.
• Excellent communication and collaboration skills.
• Flexibility and adaptability in a dynamic work environment.
