    Description:

    This role offers a unique opportunity to work in a dynamic environment dedicated to providing high-quality care and rehabilitation services to frail and elderly patients.

    Role and Responsibilities:

    • Deliver high-standard assessments and rehabilitation interventions to patients in an intermediate care setting.
    • Develop and implement individualized treatment plans, focusing on maximizing patient independence and safety.
    • Work collaboratively with a multidisciplinary team, including nurses, social workers, and other allied health professionals, to provide comprehensive care.
    • Manage a diverse caseload, ensuring timely and effective care delivery for community-based and inpatient services.
    • Educate and support patients and their families in understanding treatment plans and rehabilitation goals.
    • Actively contribute to the development and review of clinical protocols and best practices.

    Requirements:

    • Diploma/Degree in Occupational Therapy or Physiotherapy.
    • HCPC Registration.
    • Experience in intermediate care or rehabilitation settings.
    • Ability to effectively manage care for frail and elderly patients.
    • Strong community experience and a proactive approach to patient care.
    • Excellent communication and interpersonal skills.

    Skills:

    • Strong clinical skills in assessing and treating a range of conditions.
    • Ability to detect the deteriorating patient
    • Ability to risk assess patient assessment, treatment and discharge
    • Ability to assess and treat motor and sensory skills as well as physical functioning
    • Ability to assess and treat cognitive and affective impairment, its impact on function, ability of the patient to maintain their health and safety
    • Ability to structure holistic assessment and the OT process within a framework that provides clear clinical reasoning for recommendations made in relation to a patient's needs and care pathway
    • Evidence of CPD
